The second annex is a two-story facility that is in overall good condition, (P21). The second story once was apartments, however they have been turned into law offices. |

Were unable to access the roof. There are no current reports of leaks. |

The first floor windows are in good condition; however, we could not get a close inspection of the second floor windows. |

The handicap accessible restrooms meet most of the ADA requirements, (P20). The sink drainage pipe and hot water pipe are insulated. There was no fire alarm or strobe light. |

Structure
|
There is cracking evident above the doorway of the WV Division of Environmental Protection, (P15). Cracks are also present in the ceiling of the stairwell leading to the second floor. |

There were no security measures in place. All entrances including the rear entrance were unlocked and easily accessible. |
